We are looking for a dynamic and passionate RN Patient Navigator to join our team!
What You’ll Do
The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for oncology patients. She/he is responsible for ensuring all patients with an oncology diagnosis receive quality and comprehensive services. He/she will coordinate patient care throughout the continuum in collaboration with the multidisciplinary team. She/he will serve as a single point of contact for patients and/or family members for the purpose of effective communicating the patient’s plan of care. She/he will serve as a clinical resource with expertise in hematology/oncology care management. He/she will serve as a liaison throughout the facility and in the community regarding services provided for this patient population. He/she will provide expert nursing care, which includes: direct clinical practice, consultation, education and research. The successful candidate will be able to:
- Help patients secure appointments across specialties, helps patients and families prepare for their visit, and provides an overview of what to expect from the visit.
- Facilitates consults with multiple specialists and connects patients to financial counseling or other care settings.
- Provides information about what to expect on the journey: understanding the importance and role of the PCP, how to handle visit with specialists, how to clarify chief complaint, importance of understanding and sharing what medications they are taking, how to verify insurance coverage for procedures
- Stays connected with patients and care teams to create a seamless journey and to follow-up on questions or issues; clarifying the plan of care, describing test results, and updating patients and clinicians on the status of the patients’ care.
- Rounds on patients assigned to their care, ensuring patient concerns are addressed, and ensuring smooth transitions between units, and participating in multi-disciplinary rounds with physicians, clinical nurse specialists, case managers, social workers, pastoral care, pharmacists, etc.
- Under the direction of physician, notifies families of changes in status (e.g. reintubation, readmission to CTICU, surgical re-exploration, cardioversion, pacemaker implantation, new onset atrial fibrillation, etc.). Provides emotional support and education to family related to such changes.
- Educates patients on medications, procedures, and post operative care.
- Assists patients and families with financial and administrative issues (e.g. FMLA paperwork, advanced directives, Medicaid, Social Security, etc.) by referring to appropriate resources.
- Serves as a link to services across the health system: social work, pastoral care, physical therapy, other specialties, ensures that care is consistent across settings from ICU to step down to home.
- Collaborates with multi-discplinary team to provide supportive end-of-life care and arrangements, utilizing support services (e.g. Pastoral Care, Social Work, Healing Services, etc.) as indicated.
- Connects with patients post-discharge to: ensure they know the healthcare system cares about their recovery, clarify that they are taking their medications, confirm they have supplies and treatments needed, and verify they are following discharge instructions.
- Ensures that the discharge summary and medication reconciliation is returned to the Primary Care Provider in a timely manner, and that the patient is receiving appropriate follow-up care.

What You’ll Bring
- Must hold or obtain current licensure as a Registered Nurse with the Idaho State Board of Nursing.
- Oncology Certification or Oncology Certification must be received within one year.
- Five or more years of clinical experience in oncology care.
- Must hold or obtain current BLS.
- Remain current with center-wide occupational education programs.
- Writing/editing skills necessary to produce appropriate documentation and communications.
- Must have an understanding of computers and applications.
- Comprehension of standard radiation therapy practices and documentation.
- Must be able to take verbal instruction and practically apply it without direct supervision.
- Must hold chemotherapy/biotherapy certification.
