JOB SUMMARY

The Peritoneal Dialysis and Hemodialysis Nurse Program Coordinator provides guidance for the peritoneal dialysis (PD) program utilizing a multidisciplinary approach to ensure quality patient care is delivered according to End-Stage Renal Disease (ERSD) and other regulatory requirements and best practice standards in a cost-effective manner. Ensures care is accessible to patients 24 hours per day and 7 days per week providing outpatient and inpatient care, training and education to patients and staff, in home visits and on call as needed to support the continuity of care while meeting regulatory, documentation, and billing requirements of the program.
This position is exempt from Career Service under the CCH Personnel Rules.
General Administrative Responsibilities
Collective Bargaining
- Review applicable Collective Bargaining Agreements and consult with Labor Relations to generate management proposals
- Participate in collective bargaining negotiations, caucus discussions and working meetings
Discipline
- Document, recommend and effectuate discipline at all levels
- Work closely with labor relations and/or labor counsel to effectuate and enforce applicable Collective Bargaining Agreements
- Initiate, authorize and complete disciplinary action pursuant to CCH system rules, policies, procedures and provision of applicable collective bargaining agreements
Supervision
- Direct and effectuate CCH management policies and practices
- Access and proficiently navigate CCH records system to obtain and review information necessary to execute provisions of applicable collective bargaining agreement
Management
- Contribute to the management of CCH staff and CCH' systemic development and success
- Discuss and develop CCH system policy and procedure
- Consistently use independent judgment to identify operational staffing issues and needs and perform the following functions as necessary; hire, transfer, suspend, layoff, recall, promote, discharge, assign, direct or discipline employees pursuant to applicable Collective Bargaining Agreements
- Work with Labor Relations to discern past practice when necessary
Typical Duties - Develops and implements the PD program including the establishment of patient care protocols, design of documentation tools, and standards of care for the patients undergoing hemodialysis and peritoneal dialysis.
- Utilizes a multidisciplinary approach, collaborating with other members of the health care team to develop an individualized plan of care for patients and leads monthly meetings to review.
- Assists in delivery of PD to patients and ensures that safety and regulatory standards of care are met.
- Develops standards of care, competencies, policies and procedures, and effective PD education for staff. Performs evaluation and makes recommendation when necessary.
- Facilitates and ensures basic hemodialysis and PD training to all nursing staff during new employee orientation.
- Provides and oversees patient/family home training and education for PD patients.
- Conducts an analysis of the appropriateness of dialysis, analyzes labs, and collaborates with others to review/revise patient care plans when necessary.
- Evaluates the patient's response to dialytic and other interventions.
- Works with supply chain department to maintain a cost-effective supplies and equipment. Monitors supply availability and usage of all necessary items for safe quality care.
- Participates in and supports quality improvement activities to include chart reviews and monitoring data for Quality Assurance, process improvement and provides periodic reports as requested. Ensures data is entered into external and internal databased for quality reporting.
- Gathers data and enters into current system, i.e. Crown Web.
- Manages budget, expenses and variances for PD program.
- Reviews contractual requirements to ensure obligations of payors and insurance companies are met.
- Validates encounters are accurately documented and billed in a timely manner.
- Conducts and oversees patient care at home assessments for PD patients prior to treatment initiative and for ongoing patient care.
- Provides on-call services for PD patient at home to triage and direct patient on management of care.
- Identifies resources necessary to support the growth of the PD program and works with leadership to budget and secure availability.
- Attends and participates in meetings, conferences, and committee discussions to support the continuity of patient care.
- Attends hemodialysis and PD education programs to maintain clinical expertise and to enhance positive patient outcomes and promote evidence-based standards of care and practice.
- Orients and trains staff supporting PD.
- Provides ongoing support to staff working with PD patient both inpatient and in the clinic.
- Performs other related duties as needed.
Reporting Relationship Reports to the Clinical Manager Ambulatory Procedures Unit (Hemodialysis and Peritoneal Dialysis) Minimum Qualifications:

Bachelor of Science in Nursing from an accredited college or university (Official transcripts must be provided at time of interview).

Licensed as a Registered Nurse in the State of Illinois.

Five (5) years of full-time nursing work experience in an acute care setting.
-
Three (3) years of full-time combined nursing work experience in Hemodialysis and Peritoneal Dialysis, with at least two years of Peritoneal Dialysis experience.

One (1) year of outpatient Peritoneal Dialysis experience.
-
One (1) year of full-time nurse manager experience.
-
Current Basic Life Support (BLS) certification accordance with the American Health Association or able to obtain within two (2) weeks of hire. (Must provide proof of certification at time of interview if already obtained.)

Position may require moderate travel for which the employee must have a valid driver's license/insured vehicle or other equivalent means of transportation for work.
Preferred Qualifications

Experience in Medical/Surgical and Intensive Care Unit nursing experience.

Certified Nephrology Nurse (CNN)
