The Home Based Primary Care (HBPC) program provides comprehensive, interdisciplinary primary care services in the homes of Veterans living within the HBPC program service area with complex and chronic, disabling diseases. The program targets frail, chronically ill Veterans requiring care by an interdisciplinary health care team to maximize the Veterans' function and quality of life. HBPC Psychiatric Advanced Nurse Practitioner is an independent care provider who is highly skilled in the evaluation and treatment of mental health needs, and serves as a core member of the HBPC Interdisciplinary Team. The Advanced Nurse Practitioner will provide primary mental health care for HBPC patients at least 50% of total available clinic time. The Specialty Mental Health clinic provides primary mental health care to Veterans in an outpatient clinic setting. The Advanced Nurse Practitioner will also work in outpatient mental health clinic no more than 50% of total available clinic time. - The individual must possess the interpersonal and clinical skills to provide primary mental health care. - Conducts history, physical exams and mental status exams on assigned patients. - Completes intake assessments and formulates diagnosis. - Develops and presents treatment plans to the multidisciplinary treatment team - Orders all necessary laboratory and radiological assessments - Initiates consults and referrals as indicated - Provides psychotherapy and pharmacotherapy as indicated in the treatment plan - Maintains effective and up-to-date documentation of patient encounters - Participates in professional and administrative activities as assigned. Work Schedule: Monday - Friday; 7:30am - 4pm; no weekend or evenings Compressed/Flexible: yes; compressed available with supervisor approval Telework: NOT eligible Virtual: This is not a virtual position. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
