Job Description

Outpatient Telephone Triage (MSK Commack)

We Are:

The Ambulatory Care Nursing Team is expanding the hours of providing telephonic triaging of MSK patients in the home setting. The nurse will assess the severity of the health problem and determine the level of care needed. This may include advising the patient on self-care measures to remain at home or having them seen in one of MSK's Regional Symptom Care Clinics, NYC-based Urgent Care Center, or the Local Emergency Department. The Triage Clinical Nurse will use his/her oncology expertise and critical thinking skills to address the immediate needs of the patient, ensure appropriate next steps are taken, relay proper instructions and document the event in the MSK medical record.

You will:

• Utilize the nursing process to triage and manage patient symptoms via telephone and other electronic devices.
• Collaborate with physician on-call based on clinical assessment and need.
• Serve as the clinical expert and nurse consultant to provide guidance, coaching and education to patients and care givers.
• Triage and direct patient to the appropriate care site including the Symptom Care Clinic, Urgent Care Center or local Emergency Room.
• Facilitate departmental/institutional initiatives and implementation of new workflows and tools to improve care coordination.
• Advocate on patients' behalf serving as expert problem-solver with and for patients, caregivers, and their care teams to anticipate potential issues, remove obstacles, and ensure expectations are met expeditiously.
• Document pertinent clinical information from calls, interactions, and clinical reviews succinctly to ensure clarity and continuity for patients.

You need:

• New York State RN license
• Bachelor of Science degree in Nursing (BSN) required, master's degree (preferred)
• Current national nursing certification preferred.
• 2+ years of clinical nursing experience in Oncology required
• Demonstrated exceptional critical thinking skills
• Demonstrated self-direction in establishing priorities
• Strong organizational and analytical skills
• Active listening skills
• Proficient computer skills to ensure accurate, timely and complete documentation.

Hours: Monday - Friday 9a to 5pm at MSK Commack
