Nurse Practitioner (Geriatrics-Home Based Primary Care)
The Nurse Practitioner is assigned to the Home Based Primary Care Program as an Interdisciplinary Team Member and Primary Medical Provider. Treatment modalities include those of a Primary Medical Provider, inclusive of standard medical care that is offered in a safe, effective, efficient, and compassionate manner. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
