Schedule: Full-Time, 40 hours per week
Shift: Dayshift, Monday -Friday 8:30pm-5pm with Manager on call rotation.
Coverage Area: Cover Inpatient Unit in Flagler and Volusia County.

The role you’ll contribute:
As part of the HospiceCare leadership team, the IPU Coordinator directs and supervises the day to day patient care delivery by members of the hospice interdisciplinary team.In conjunction with the patient, family and the patient's attending physician, he/she develops and manages the patient's plan of care.The IPU Coordinator ensures that care delivery is in accordance with Medicare Hospice Conditions of Participation and within the organization's budget and productivity guidelines.The IPU Coordinator acts as the clinical resource for staff members, and facilitates weekly interdisciplinary team meetings to discuss the patients' plans of care and ongoing eligibility for hospice services.The IPU Manager is responsible for clinical productivity, education and appropriate documentation by staff to meet the Medicare COPs for Hospice.The IPU Coordinator is responsible for staff hiring, orientation and productivity.The IPU Coordinator is responsible for staffing each unit and oversight for all compliance per Hospice regulations.The IPU Coordinator reports to the Assistant Director of Clinical Services

Qualifications
EDUCATION AND EXPERIENCE REQUIRED:
·Registered Nurse
·Critical nursing
·2 years’ experience in hospice or end of life care
·Supervisory experience
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
·Registered Nurse
·BLS
·Florida Driver’s license and auto insurance
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
·CHP
